RN Case Management
Mom and Dad are hours away. I provide eyes on your senior loved ones 2–3 times a week, coordinating your full care plan — whether recovering after a hospital stay, managing a chronic condition, or aging in place safely at home.
Medication Management
I review your physician's orders to ensure you are taking the right medications at the right time and in the right dose, reducing the risk of medication errors.
Care Navigation
I assist with coordinating transportation to appointments, grocery stores, and social activities so you stay connected to what you love and need.
Insurance Coordination (Private and Military Veterans welcome)
I work directly with your physician's orders and help navigate private and Veteran insurance coverage.
Daily Planning & Independence
I assist you in maintaining as much of your daily routine as possible — collaborating on nutritious meal plans, following your therapist's plan for occupational independence, and establishing a healthy routine.
Transitional Post-Hospital Care
Being discharged from the hospital can be overwhelming. I provide nursing care for joint replacements, shoulder or ankle repairs, and cosmetic surgery recovery — guiding you every step until you are healed and released by your physician.
Wound Care
I provide individualized wound care plans based on your physician's orders, treating complex and chronic wounds in the comfort of your home — whether that's a house, apartment, relative's home, or senior living community.
Traveling Assistance
I help with grocery lists, running errands, purchasing supplies, accompanying you to doctor's appointments, and completing tasks that may be difficult to manage on your own.
